Waiver of Liability: By my selection below, I acknowledge that there are inherent risks and dangers associated with recreation programs, and therefore, I hold Frederick County Maryland, and its agents harmless from all claims for injuries, damage, or loss which may result from my, or my child(ren)’s, volunteering on behalf of the Recreation Council. Further, in compliance with Maryland HB 858 and SB 771, I hereby acknowledge that I understand the risks of concussions in sports and am aware of the concussion information which is found at www.reccouncilsoffrederick.org. *

Background Check: In connection with my application to become a volunteer with the Recreation Council, I authorize any person, business entity or governmental agency that may have information relevant to the above to disclose the same to the Frederick County Division of Parks and Recreation, including, but not limited to, any and all courts, public agencies, law enforcement agencies and credit bureaus. I authorize the County to share such information only with parties in interest who have a “need to know” such information to protect them and their volunteers/participants/employees. The Recreation Council does not sell or otherwise provide any of the information found in its background investigations to any other party.*

Authorization for use of Photographic Likeness: I agree to allow Frederick County Parks and Recreation and the Recreation Council to take and utilize photos, slides, and video images of the above individual(s) for the purpose of promotion and publicizing of the Council's leagues and/or events.

1) I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice, or other youth sports event.

2) I will place the emotional and physical well-being of a child ahead of any personal desire to win.

3) I will insist that we always play in a safe and healthy environment.

4) I will have knowledge of basic first aid principles and will never knowingly allow an injured player to continue participation without proper medical approval.

5) I will ensure that all equipment used by my players is safe and conforms to the rules of the game. I will not allow illegal or unsafe equipment to be used.

6) I will provide a sports environment that is free of drugs, alcohol, tobacco, and abusive language and will refrain from their use at all sporting events.

7) I will remember that the game is for children and not the adults.

8) I will ensure that my players are supervised at all times.

9) I will expect my players to treat other players, coaches, fans, and officials with respect regardless of race, sex, creed or ability.

10) I will be knowledgeable of the rules and techniques of the sport that I coach and I will teach these rules to the players on my team.

11) I will do my best to have organized and structured practices that challenge players of all ability levels.

12) I will always be fair and treat everyone equally when discipline is required.

13) I will never knowingly allow an ineligible player to participate on my team or withhold information from another coach about a potentially ineligible player on my team.

14) I will always be available to discuss any issues with parents concerning their child in a private environment.

15) I will do my best to make youth sports fun for the children and will encourage teamwork and camaraderie.

I understand that volunteering for the recreation council is a privilege and that any actions that I take that violates the council’s code of conduct may result in my removal as a volunteer*
