Background Check: In connection with my application to become a volunteer with Frederick County Parks and Recreation, I authorize any person, business entity or governmental agency that may have information relevant to the above to disclose the same to the Frederick County Division of Parks and Recreation, including, but not limited to, any and all courts, public agencies, law enforcement agencies and credit bureaus. I authorize the County to share such information only with parties in interest who have a “need to know” such information to protect them and their volunteers/participants/employees. Parks and Recreation staff does not sell or otherwise provide any of the information found in its background investigations to any other party.*

Further, in compliance with Maryland HB 858 and SB 771, I hereby acknowledge that I understand the risks of concussions in sports and am aware of the concussion information which is found at www.recreater.com. *
