  2. LEAGUE PARTICIPATION WAIVER AND PARENT'S CODE OF ETHICS AGREEMENT

    I hereby pledge to provide positive support, care, and encouragement for my child participating in youth sports by following this Parents’ Code of Ethics: 1) I will encourage good sportsmanship by demonstrating positive support for all players, coaches, and officials at every game, practice or other youth sports event. 2) I will place the emotional and physical well being of my child ahead of my personal desire to win. 3) I will insist that my child play in a safe and healthy environment. 4) I will support coaches and officials working with my child, in order to encourage a positive and enjoyable experience for all. 5) I will demand a sports environment for my child that is free from drugs, tobacco and alcohol and will refrain from their use at all youth sports events. 6) I will remember that the game is for youth - not adults. 7) I will do my very best to make youth sports fun for my child and his/her teammates. 8) I will ask my child to treat other players, coaches, fans and officials with respect regardless of race, sex, creed or ability. 9) I will help my child enjoy the youth sports experience by doing whatever I can, such as being a respectful fan, assisting with coaching, or providing transportation.

  3. WAIVER OF LIABILITY

    By my signature below, I acknowledge that there are inherent risks and dangers associated with recreation program/s and therefore, I hold the Board of County Commissioners of Frederick County (BoCC) harmless from all claims for injuries, damage, or loss which may result from me or my child’s participation in the program/s listed above. Further, in compliance with Maryland HB 858 and SB 771, I hereby acknowledge that I understand the risks of concussions in sports and am aware of the concussion information which is found at www.recreater.com

  4. AUTHORIZATION FOR USE OF PHOTOGRAPHIC LIKENESS

    I agree to allow Frederick County Parks and Recreation to take and utilize photos, slides, and video images for the purpose of promotion and publicizing of the Division’s programs, facilities, and/or events.

  5. CODE OF CONDUCT

    I understand that the Division has a discipline policy for conduct in recreation programs and facilities. I agree to follow the Division’s Code of Conduct. In the event I/my child violates the Code of Conduct and I/my child and asked to leave the program or facility, I understand that the registration fee will not be refunded.
